Please register for the 2009 -20010 DHSI Grand Rounds Series by completing this form, and be sure to also sign in at each seminar you attend. In accordance with OCME regulations, physicians may only receive a certificate of credit for seminars where their attendance can be confirmed by their signature on the sign-in sheet for each event. Non-physicians will also need to register and sign-in in order to receive a certificate of attendance

CME credit is now available for those attending the seminars through our Webcast by following these procedures:

  1. Complete and return the annual registration form below.
  2. Send an email to confirm you are online at the start of the Webcast to: informaticsseminar@jhmi.edu
  3. In a follow up email to be received within an hour of the conclusion of the Webcast, briefly answer some questions about the seminar's objectives and complete a short evaluation of the session.
