REGISTRATION PROCESS and GRADES

Please note that DHSI students MUST register through the School of Medicine, even for courses in other divisions. Our contact there is Gayle Miller and she will work with you to process interdivisional registrations. Regardless of your pre or post-doctoral status within the Division, you should always identify yourself as a graduate student for registration purposes. If you have a problem, even with interdivisional registration, please contact Gayle Miller and notify the program coordinator. Note that ALL registration forms are available at 147BRB and not in the coordinators office.

Fall and Spring: Submit a Schedule Change form signed by the program Director AND a green interdivisional form (if applicable) On the green sheet there is a box to mark your classification. You are GRADUATE student, 31 = 1st year and 32= 2nd year.

Summer and Winter: Change of Schedule form AND Verification of Enrollment form. Students who register for interdivisional courses in the summer and who take Winter Institute courses at Public Health during January need to use a Verification of Enrollment form which they get from the registrar’s office. These forms act as cash since the divisions bills SOM directly for tuition for these courses. Students are given the forms and must take them directly to the correct divisions to register.

Online Courses: Please note that all online courses have Introduction to Online Learning as a prerequitiste. This course does not take long to complete and is offered quite regularly, but do be sure to have finished the Introduction to Online learning first or you will be shut out of any online courses for which you have registered. ADD/DROP POLICY - you MUST add and or drop a course by the mid point. Failing to drop a course by the deadline could result in an F on your official transcript. Please contact Gayle Miller in the SOM registrar's office with questions.

GRADES- At most, two courses with grades less than B– may be counted towards the coursework requirements. No course with grades less than C- may be counted. The overall grade point average of the courses counted towards the coursework requirements must be a 3.00 or higher (B average). At most, two independent study courses can be counted towards the course requirements. No courses with grades of S can be counted towards the coursework requirement. Courses with grades of S will not be included in the grade point average calculation.

A grade of D or F in a non core course will result in probation; a second D or F would be cause for dismissal from the program. If the course is a DHSI core or other required course a grade of C or below will result in probation and all other rules apply.

P/F and Course Auditing: When signing up for non core courses please keep in mind that you still should be taking them for credit. Courses that you wish to count toward your program MUST be taken for a grade. Pass/Fail and Auditing are options only for courses that you will not count toward the degree and must be approved by the program director. Also see grades policy below.

Independent Study: Independent Study courses must be approved by the Director. DHSI/SOM registration and grading policies apply.

Process: The student must submit a course description to the Training Program Director, Course Instructor and Program Coordinator. This description will include length of Independent Study (up to 2 quarters or 1 semester), the time requirements (i.e 3 hours a week per quarter), the student’s goals and what the deliverable will be. Once the Program Director accepts your proposal, the Coordinator will supply you with a course number to register for. The course instructor must be clear that a grade needs to be submitted on their department letterhead to the program coordinator. The Program Director will then sign off on the course roster.

Probation: Upon being placed on probation an email and official letter from the program director will be sent. A formal meeting with an ad hoc subcommittee, comprising at least three members of the Executive Committee (of the Director’s choosing), will be scheduled. During the meeting the student will have the chance to review the grade, to explain what was done in response, and to develop consensus on a plan to monitor progress in going forward with the program, including a time line. The notification will become part of the student's permanent record, as will documentation of success in overcoming it.

Upon review of progress later by the ad hoc subcommittee, inadequate progress will be grounds for dismissal from the program.

Progress Tracking: It is the responsibility of the fellow to track their progress throughout the fellowship. At the end of every quarter you are responsible for reviewing your progress with the program Director. The coordinator will email out the OFFICIAL (on file) version of your record a week or two before the Quarter ends. You MUST update your progress and mail it to the Director for review 3 days before your scheduled meeting. After it is discussed with the Director, he will forward the revised document to the Coordinator, making it OFFICIAL. The School of Medicine registrar sends a copy of your full year registration for review annually. It is your responsibility to review the file and check for discrepancies.
